Wakefield Services
Wakefield Health Inclusion Team (HIT) is a service commissioned to provide health interventions, screening, assessment, support and signposting for clients that fall within the recognised health inclusion categories (under core20+5).
Examples of the communities we serve include (but not exclusive to):
- Gypsy, Traveller, and Roma
- People experiencing homelessness or vulnerably housed
- Vulnerable Migrant
- Sex Working
- People recently released from custody
- People experiencing addiction
The service operates within the communities through attendance at Homeless shelter, Soup Kitchens, Volunteer/Charity Sector locations, HMO accommodation, on the street etc.
Wakefield HIT primary focus is to eliminate the barriers to accessing healthcare on an individual basis, modelling a biopsychosocial and trauma informed approach with each patient interaction and making every contact count.
