Bevan operate a two pronged approach to improving outcomes for vulnerable groups who go to A&E or who are admitted to hospital.
Together Pathway and BRICCs provide compassionate health and wellbeing care to improve the life outcomes for vulnerable groups whilst also delivering cost effective interventions which reduce the cycle of readmission and bed blocking and so helps to improve overall hospital capacity.
The teams have been recognised by Emmaus to who recognised their work with an Award for Outstanding Contribution and The Queens Nursing Institute who published a Best practice / Innovation Case Study of the team’s work. As a result, the team presented at the European Federation of National Organisations Working With the Homeless Conference.
