Inclusive Health and Wellbeing at Bevan Healthcare

Bevan Social Prescribers

Bevan employ a team of social prescribers who can work with people to help them access additional support to improve their health and wellbeing. Many of the people a Social Prescriber supports would like to make new friends or find out about local activities.  They may have experienced trauma or lost family, have housing or money issues, had a new baby or are struggling to come to terms with a long-term medical condition.

The Social Prescriber will meet with the individual one to one in order to discuss the type of support they need.  A plan of action will be discussed, and they may also be referred or signposted to other services either within Bevan or to community groups or partner organisations we work with

If you would like to find out more about the service, you can contact the Social Prescriber directly on 01274 905323 or email bevanwellbeing@bradford.nhs.uk . Alternatively, you can drop into the Wellbeing Centre reception on Dale Street in Bradford or ask your doctor.
